BMW Plant 10 is pleased to offer BMW International associates and their families (age two-years and above) onsite health care at the BMW Associate Family Health Center (AFHC). To be eligible to receive services, the Associate or covered family member must have the Hallesch/Henner insurance card. The Associate Family Health Center and Pharmacy, operated by Premise Health, is committed to protecting your medical confidentiality rights and privacy. The Associate Family Health Center and Pharmacy, and Premise Health will maintain the privacy and confidentiality of your personal health information and medical records in compliance with all applicable state and federal laws.

ELIGIBILITY
The Associate Family Health Center and Pharmacy is open to Associates and their spouses and dependent children age two and older who are enrolled in a BMW health care plan for medical, dental, and vision coverage.
