The Associate Family Health Center is your medical home.  We would like it to be the first place you think of for all your healthcare needs including prevention, wellness, acute care, and chronic care.  A PCMH offers you personalized care plans, medication review, coaching, advice, and support.  The Associate Family Health Center care team will guide your way and coordinate your care as you make your journey to better health.

A PCMH is not a building, a house or a hospital.  It is a partnership with your primary care provider; relationship based, with an orientation toward the whole person.  PCMH staff work with each patient to develop a personalized care plan that respects each patient’s unique needs, culture, values, and preferences. Your primary care team will help you navigate all of the elements of the complex healthcare system and provide the right care at the right time by connecting you with community specialists and resources when you need them.


Health Center Primary Care Services

The Associate Family Health Center primary care offers a wide range of convenient and affordable services typical for family medicine. Our services include wellness physicals, chronic disease management and acute care needs. We have extended office hours as well as limited Saturday morning hours for sick visits. We want to be your first stop for a diagnosis, acute sickness, prescription refill, annual physical, immunization, or lab work.

The center is equipped to handle a variety of personal medical services for adults and children age two and older, including:

  • Immunizations

  • Lab services

  • Digital x-ray services

  • Care management

  • Nutrition services

  • Primary and acute care:

    • Urgent care visits (for acute illness, or infections, cuts, sprains, strains, minor illnesses and injury, minor surgical procedures)

    • Routine office visits

    • Preventive / annual physical exams

Whether you schedule a physical exam in advance or experience an acute care need, you can choose to use the Associate Family Health Center as your family’s medical “home”, or use it to supplement your current doctor’s care — it’s your choice.


General Wellness

The Associate Family Health Center offers a comprehensive approach toward wellness, preventive care and chronic conditions, where the focus is on you — your health and what it takes to have optimal health and maintain it. 

We offer several wellness programs for Associates and their family members:

  • Lower to Live Program for high blood pressure

  • The Dimensions Program for diabetes

  • The Cholesterol Program for high cholesterol

  • Lifestyle Coaching including weight management, tobacco cessation, and nutrition

If your medical condition requires the involvement of a specialist, we will make referrals and continue to assist with coordination of care as needed for our patients. We also coordinate with other departments to make sure you receive the optimum care needed for your overall health. In addition, we offer integration with health and wellness coaching to help our patients feel empowered to manage their health. 


Health Center Staff

Our clinical staff consists of Board Certified Physicians, Certified Nurse Practitioners (NP’s), Physician Assistants (PA’s), Licensed Nurses, Certified Medical Assistants, and Registered Radiologic Technologists. All are qualified and compassionate professionals here to provide the utmost care for our patients.

Daren Marionneaux - Medical Director

Our medical director, Dr. Daren Marionneaux, MD, came to us from Augusta, GA. He received his undergraduate degree from Eastern Kentucky University, and graduated with High Distinction from the University of Kentucky College of Medicine. He did his residency and internship at Dwight D. Eisenhower Army Medical Center in Ft. Gordan, GA and has practiced in the military, the VA, and in both solo and private practice settings. He is board certified as a Diplomate, American Board of Family Medicine.

Jeremie Kenney - Family Nurse Practitioner

Jeremie Kenney has a wide range of experience in family practice, urgent and intensive care, and hospice. He received certification from the American Nurses Credentialing Center as a Family Nurse Practitioner. His education consists of a Bachelor of Science in Nursing and a Masters of Nursing from Texas Woman's University.

Jesse Williams - Physician Assistant

Jesse has a  Masters of Physician Assistant Studies, from South University, Savannah, GA. His vast experience includes Family Practice, Emergency Medicine and Vascular Surgery.  He is certified with The American Association of Physician Assistants and active in the South Carolina Association of Physician Assistants and National Commission on Certification of Physician Assistants.

Elizabeth Scarborough - Physician Assistant

Elizabeth Scarborough has her Masters of Physician Assistant Studies, from the Medical University of South Carolina in Charleston, SC. She has experience in Family Medicine and enjoys treating children and adults through their lifespan. She has a Bachelor of Science from Clemson University and graduated Magna Cum Laude from the Calhoun Honors College. She is certified with the National Commission of Certification of Physician Assistants and is active in the South Carolina Association of Physician Assistants and American Academy of Physician Assistants. 


Jacob Davis - Health Center Group Manager

Jacob Davis is the Health Center Group Manager at the BMW/Premise Health Associate Family Health Center in Greer, SC. Jacob has his master’s in healthcare administration and has worked in a wide variety of healthcare settings over the last 12 years. Jacob is passionate about serving others and looks to continue his service with BMW and Premise Health.


Cate Milne - Dietitian Supervisor

Cate is a Registered Dietitian Nutritionist and wellness coordinator. She holds a Bachelor of Science in Food Science, Nutrition, and Health Promotion from Mississippi State University. She is also a certified health coach with a background in personal training. She previously worked at an inpatient acute care facility where she provided specialized nutrition support therapies to patients. She now practices at the AFHC with the goal of helping individuals achieve overall well-being and health as well as prevent and manage chronic conditions.


Angela Ballard - Certified Occupational Health Nurse Specialist

Angela has over 25 years’ worth of experience as a nurse. She received her BSN from USC Upstate and her MSN in Nurse Administration / Nurse Education from Gardner-Webb University. She also holds certifications including COHN-S – Certified Occupational Health Nurse Specialist, and also COHN – Certified Occupational Hearing Conservationist. Her experience and knowledge helps to provide the most up-to-date guidance and supervision for the Occupational Health Department at the AFHC.


 

Brooke Wilcox - Care Manager Supervisor

Brooke Wilcox is a Registered Nurse with a Bachelor of Science in Nursing (BSN) from the University of Arkansas. Her nursing experience includes outpatient hemodialysis, behavioral health, and chronic condition management. She is currently working as a Condition Management Nurse at the AFHC, helping individuals reach their full health potential through education of lifestyle interventions and chronic condition management.

 

 

Genesis Kiser - Primary Care Manager

Genesis Kiser is registered nurse with eight years of experience in the healthcare field. She obtained her Bachelor of Science in Nursing from Charleston Southern University. Her passion for nursing led her to specialize in Neuro Trauma ICU at a level 1 trauma center. She then transitioned into Occupational Health and primary care.
